Return Policy


1) All requests for return product should be emailed to Support@MidwestMD.net. No returns will be accepted without an RGA number, and only those products and quantities approved should be returned.

2) Products returned within 30 days will not be assessed a restocking fee. Products returned after 30 days will be assessed a 25% restocking fee. Products that have shipped over 60 days can not be accepted as returnable.

3) All return shipping costs are at the expense of the customer, UNLESS the product is damaged upon receipt.

4) SURGEON EVALUATIONS - All surgical instruments are available for visual evaluation. Evaluation surgical instruments are invoiced at the time of shipment. Undamaged, unused surgical instruments may be returned for full credit within 30 days. Evaluation surgical instruments which have been autoclaved or used in surgery cannot be returned. Please contact us regarding the availability of trial use requests. Specialty manufactured surgical instruments may not be returned.

In the event a return is requested, please provide the following:

  • Invoice number & P.O.
  • Returns will not be accepted without a RGA number.
  • Returns should be made within 30 days of RGA number issuance.
